Wisconsin's two big public programs for young children with delays and disabilities both run through your county — which means the right phone number depends entirely on where you live. This page puts all 72 counties in one place.

Birth to 3 is Wisconsin's early intervention program for children under age 3 with delays or disabilities — no diagnosis needed to be evaluated. The Children's Long-Term Support (CLTS) Waiver is a Medicaid program that funds extra supports for children with disabilities through county health and human services agencies. How this list was built, honestly: the CLTS phone number for every one of the 72 counties comes straight from the official DHS county contact list. Birth to 3 contacts live in a DHS search tool that can't be reprinted reliably, so each county links to the official directory instead — with two exceptions, Milwaukee and Waukesha, whose referral lines we verified on the counties' own websites. Where DHS lists a separate number for new applicants, that's the number shown. Staff and numbers change; the county will point you to the right desk.
