What do you wish someone had explained sooner?

Fathom North is a pediatric therapy clinic being built in Oak Creek. The walls aren't up yet — which means the part of the clinic no architect draws, what it feels like to be a parent here, is still being designed. We'd rather design it from your experience than our guesses.

Here's our end of the deal: a person reads every response — every word. We'll tell the Founding Families list what we heard, and we'll let it change what we build. Not a survey. A conversation we intend to hold up.

Was there a moment intake or enrollment anywhere felt confusing or cold?What made it feel that way? While you waited, what did you actually want from the provider during the silence? Has any provider ever made you feel like a partner instead of a spectator?What did they do? What question were you afraid to ask — or didn't know you were allowed to?

Please leave out your child's name, date of birth, diagnosis, school, medical or treatment history, insurance details, and provider names unless the story truly needs one — we don't need any of it, and we'd rather you kept it. We're listening for the experience of navigating care, not clinical histories.

How we handle what you write

Why we're collecting this: to shape how Fathom North is designed — the family experience, the communication, the things nobody explains. Nothing else.
Who reads it: the small team designing the family experience. A person reads every response.
Where it goes: your response is stored with our website's form submissions and reviewed by that team. It is not a clinical record, and submitting it doesn't create one.
Email is optional. Anonymous is genuinely fine — anonymous responses inform the themes we learn from, and are never quoted.
What we may share publicly: de-identified themes — what families in general told us. We will never publish your exact words without confirming the exact wording with you in writing first.
